Care Partners and Cook County Higher Education present Lunch & Learn “Understanding POLST (Provider Orders for Life-Sustaining Treatment)” on Wednesday, October 28, from noon to 1 pm at CCHE or over Zoom. In this session, Dr. Kurt Farchmin, geriatrician and former Sawtooth Mountain Clinic provider, will help participants understand one of the most frequently misunderstood Advance Planning documents – the POLST form.
