An Age-Friendly Community Assessment Need Survey for insights is available online at carepartnersofcookcounty.org. Paper copies are also available at Cook County Public Health and Human Services, Grand Marais Library, Sawtooth Mountain Clinic, and the HUB in Grand Marais.
